2022 death and licensing dispute

Utah regulators scrutinized Maple Lake Academy after a 14-year-old girl died there in January 2022. The Salt Lake Tribune reported in 2024 that state licensers had decided not to renew the girls program's license over continued safety concerns, and that Maple Lake Academy leadership blamed the closure on a more punitive regulatory approach.
