2019-2020 Disability Rights Ohio investigation

Disability Rights Ohio investigated Sequel Pomegranate Health Systems over conditions in its residential treatment facility. Its public report described concerns including unapproved restraints, injuries, staff conduct, an unstructured environment, and a grievance process that the organization said did not adequately protect children's rights (Disability Rights Ohio).

The report said Disability Rights Ohio could not conclude after nine months that protections for children in treatment had improved. The facility and its attorneys are described in the report as responding to specific concerns, including plans for staff retraining and policy development.

2021 license relinquishment

The Ohio House reported in 2026 that, following Disability Rights Ohio's investigation, the state suspended admissions at Sequel Pomegranate Health Systems in Columbus. The same account said the facility later changed its name and voluntarily relinquished its license in 2021 (Ohio House of Representatives).
